About the OfficeMax survey site:
OfficeMax are offering customers the chance to take a customer survey and in return they will get a $5.00 off coupon on their next visit to the store. Please note that $25.00 must be spent in order to obtain the $5.00 discount. So if you spend $30 the total bill should come to $25.00. To take the survey you must have the survey code, the date of visit and the time of purchase. All of this information can be found on your OfficeMax receipt. The survey code will be 12 digits long and please do not include the dashed when entering it into OfficeMaxFeedback.com. The survey is powered by the Service Management Group and should take less than 5 minutes to complete. There does not appear to be a required time frame in which to take the survey but we recommend you do it within 72 hours of your visit just to be on the safe side.

OfficeMax specializes in office supplies, office furniture and other types of office technology. The first store was opened in 1988 by Bob Hurwitz and Michael Feuer in Cleveland OH. OfficeMax is currently based out of Naperville, IL and can be found on the New York Stock Exchange under ticker symbol OMX. They have over 30,000 employees and have close to 1,000 locations nationwide. Some popular items found at this office supply store include ink and toner cartridges, printing paper, desk chairs, laptops and laser printers. Prices tend to fall slightly above average for office store supplies. If you’re looking to save a buck you might want to try Wal-Mart instead of OfficeMax (but the you have to deal with the “people of Wal-Mart”).
