About the Perkins survey site:
Perkins is giving customers 3 days (or 72 hours) to take the Perkins Customer Survey online for a chance to win $500. A customer will need the 15 digit survey code number in order to start the survey. The code can be found near the bottom of your Perkins receipt. It is suggested you keep the receipt or write down the code before discarding it in the trash can. If a customer cannot access the Internet the survey can be taken over the phone by calling the toll-free number found the receipt. Like online the phone method must be completed within 3 days of the dining experience. No purchase is required to enter the $500 sweepstakes. Customers are limited to only one survey per customer per month and the $500 sweepstakes is not open to employees of Perkins Restaurant & Bakery.
